MOKWHEEL E-BIKES — TEST RIDE WAIVER AND RELEASE OF LIABILITY
Assumption of Risk and Release of Liability
I, the undersigned, wish to participate in a test ride of an electric bicycle ("E-Bike") provided by Mokwheel E-Bikes ("Company"). I understand and agree to the following:
Voluntary Participation: I am voluntarily participating in this test ride and understand that riding an E-Bike involves certain inherent risks, including but not limited to collision, falls, loss of control, or injury due to terrain, traffic, weather, or mechanical failure.
Assumption of Risk: I freely and knowingly assume all risks associated with riding an E-Bike, including the risk of injury, property damage, or death.
Condition & Equipment: I agree to inspect the E-Bike prior to riding and confirm it appears to be in good working condition. I will wear a helmet and follow all instructions provided by Mokwheel staff.
Rules of the Road: I agree to operate the E-Bike in a safe, lawful manner and obey all local traffic laws, signals, and regulations during my test ride.
Health & Ability: I certify that I am physically able to operate the E-Bike and do not have any condition that would impair my ability to do so safely.
Release of Liability: In consideration for being allowed to test ride a Mokwheel E-Bike, I hereby release, waive, and discharge Mokwheel E-Bikes, its owners, employees, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may occur during or after my test ride.
Indemnification: I agree to indemnify and hold harmless Mokwheel E-Bikes from any claims made by third parties arising from my actions or negligence during the test ride.
Use of Image: I consent to the use of photos or videos taken during my test ride for marketing or promotional purposes, unless I opt out using the checkbox below.
Acknowledgment: I have read this waiver carefully and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Mokwheel E-Bikes, and I sign it of my own free will.
